  • Mental status: MMSE (if the patient is not in delirium) and aphasias, level of consciousness.
  • Cranial nerves examination.
  • Motor system: inspection, tone strength, coordination, power grading (1 to 5), reflexes (grading), Babinski response, gait and posture.
  • Cerebelar testing: Romberg, standing balance, pointing, tremor.
  • Sensory system: pain and temperature, position and vibration, light touch, discriminative sensations, common dermatomes abnormalities, temperature assessment, standard assessment of peripheral sensation (10gm filament), vibration, position (start with toes). Compare left and right for all of the above.
  • Ophthalmology examination.
  • Special tests: primitive reflexes, meningitis.
  • Correlate clinical findings with neurophysiology (EEG, EVP, EMG).
