When transitioning care to another individual, the student will provide feedback to the person giving handover to ensure informational needs are met and asks clarifying questions as needed. They will communicate with the health care team and patient (family/caregiver/advocate) that the transition has occurred.

The student should be able to perform handover or make a specialist or community-based referral, that minimizes known threats to transitions of care (e.g., by ensuring to engage the listener), using a structured template for verbal communication.

Students should recognize patients at risk of immediate deterioration and mobilize appropriate resources urgently. The student will evaluate and provide initial management for an unstable patient with an obstetric or gynecologic presentation and seek appropriate assistance early on in the patient evaluation process.
