Child Objectives
  • When transitioning care to another individual, the student will provide feedback to the person giving handover to ensure informational needs are met and asks clarifying questions as needed. They will communicate with the health care team and patient (family/caregiver/advocate) that the transition has occurred.
  • The student should be able to perform handover or make a specialist or community-based referral, that minimizes known threats to transitions of care (e.g., by ensuring to engage the listener), using a structured template for verbal communication.
  • The student should be able to generate succinct communication that conveys, at a minimum, illness severity, patient demographics and wishes regarding care, a concise medical history, current problems and issues, pertinent and/or pending laboratory, radiological and other diagnostic information, situation awareness, action planning, and anticipatory guidance.
