Carlos Sepos, a 40-year-old paramedic, presents to the emergency department with a two-day history of sudden onset of pain and swelling of the right knee. This has never happened before, and no other joints are affected. Carlos returned from a trip to Thailand with his buddies about 5 days ago. He recalls bruising his knee when he fell during a hike. Past medical history is unremarkable. He wonders if he has gout as his 66 year-old father suffers with this.
