Define "Frailty" in the context of the following common problems as they contribute to functional status decline in the elderly including: Incontinence, Falls, Polypharmacy, Depression, Cognitive impairment (FM approach). Identify important topics of discussion with the elderly patient and family/caregiver meetings including: Advance care planning and Goals of Care discussions, Caregiver burnout, Driving, Community resources, Transition planning.
