Ms. Broyer is a 42-year-old female who has moved to the local area and enrolled in your practice. Her past medical history is significant for hypertension and type 2 diabetes, with newly diagnosed HFpEF. She was recently assessed in the emergency department after presenting with orthopnea and increased edema. An echocardiogram was done, showing left ventricular hypertrophy with LVEF 60%. A CXR confirmed the presence of pulmonary edema. She was diagnosed with heart failure and initiated on furosemide 40 mg daily in addition to her prior medications of metformin 500 mg BID, indapamide 2.5 mg daily, empagliflozin 25 mg daily.
